Should I change treatment?

I have had plaque psoriasis for over 12 years

I am 21 years old and have had severe plaque psoriasis from the age of around 10. By around 12 years old I found steroid creams that worked, and I have been using Dovobet ointment, Synalar gel and Diprosalic application since then. At around age 15 the psoriasis was still covering around 60% of my body and so I started on Ciclosporin, and it worked very well (in conjunction with the steroid creams) for a long time. The problem is, this last year I've noticed that I've needed more Dovobet each time and I seem to be having more flare ups than I used to, even though I'm doing exactly what I've always done. So at my dermatologist today he suggested switching from Ciclosporin to Methotrexate. Of course I would love it if Methotrexate helped me but I'm feeling slightly scared of the side effects. Also, and this will sound slightly pathetic, but one of the reasons my psoriasis can affect me emotionally is because it hinders my social life massively, therefore if I start taking Methotrexate then won't my social life be worse since I can't drink on it? Hi, I think your concerns are appropriate. Have you tried the UV light treatment? Considering your age I think changing to something as drastic as what your doc has suggested could make your skin worse (due to stress and worry). I think you neec to explain youd like to try the other options available and see if those help, ie try the creams that used to work along with light treatment. Hope it helps :)

Hello Maddi Sorry to read you're having such a difficult time with your psoriasis. Many people find methotrexate works really well for them and that's great. I'm with you though and have yet to use it - despite repeated suggestions by various dermatologists. Completely understand that your skin hinders your social life. If you can, try not to let it as much as possible. Your friends and social life are as, if not more important, than your psoriasis. Being with them, relaxing, and enjoying yourself are vital for your wellbeing which will hopefully help with the psoriasis too. Couple of things concerned me in your post. You mention having used dovobet and ciclosporin for a long time. Both are great but both are intended for short term use (no more than a few months) at a time and ciclosporin not too often. It certainly isn't intended as a long term general form of management. Dovobet can be very helpful at getting psoriasis under control but as it contains a very potent steroid it should be handled with care and reduce applications in a staged manner. (Had a dreadful rebound after stopping too quickly). Wonder if it would be worth you contacting the association to have a chat about treatments. They're very helpful. See there's a post from the SF Salt Co recommending their product - what a surprise! Sea salt can be helpful and I use it regularly but in true bbc style, there are plenty of other suppliers on line. Salt from the dead sea is good, if expensive. Most health stores stock it so you could always buy a small amount to try- just remember to shower it off afterwards. Will stop now but final thought is to wonder if you could do with a break from treatments? Stick with your favourite emollient and leave it at that. Give yourself time to think about what you would like to use. There's no rule to say you have to and sometimes taking a step back to give yourself an emotional breather can be just as valuable. Apologies for length of rant but having been in your position know how rotten it is to be faced with all this at your age. All the best.
