My psorasis

Hello my name is Charley I’m 26 years old from south London England and I’m struggling with my psorasis for nearly 5 years now it’s getting worse and I get very bad skin tearing from my psorasis I have it bad on my elbows my knees and ankle what would be the best treatment can anyone recommend ?
