Recurring Guttate Psoriasis

Hi everyone, I’m new here - joined specifically to ask this question! :) In 2021 I had my first Guttate Psoriasis reaction to a strep infection following some nasty insect bites. It was pretty classic in presentation - unfortunately turned into plaque which I didn’t have before.. My now ever permanent plaque psoriasis is usually ‘controlled’ by topicals but never goes away. My NHS Derm said I don’t meet the criteria for phototherapy. A few weeks ago I got strep again. A very strange thing has happened to the palm side of my hands - GP thinks it’s a strep rash - I’m not convinced. Horrible burning sensation, rough feeling like sandpaper and incredibly uncomfortable. My hands are now peeling all over. It’s delightful! As if to make matters worse, in the last 48hrs I have all the signs of a guttate reaction again, exactly the same as in 2021. I have two questions - has anyone else had this type of experience and does Guttate now happen every time you get a nasty infection? And has anyone had better luck being accepted for phototherapy if you go private - or do they still have the 20% skin coverage rule? Sorry for my long an waffley message and thanks to anyone who takes the time to read and reply!

I had plaque psoriasis from 1980, and by around 2005 it was gone except for occasional patches on elbows and one occasional patch in my groin. In 2017 I had a total hip replacement, and a couple of weeks later I started to show itching spots which very soon became guttate psoriasis all over my body except my hands and face, strangely. My GP (I still could choose who to see back then) predicted it would fade at around 6 months, and it took 8 months but did "go away". He did give me 7 days of antibiotics at the start, just in case there was some underlying infection. Since 2017 it has not come back, despite having covid vaccinations and boosters, and also a particularly nasty chest infection late last year that took weeks to clear and required antibiotics in the end.

Thank you for taking the time to reply. I know everyone’s bodies are different but it gives me some hope that I may not need to brace myself for guttate every time I’m ill!

Overcoming Guttate Flare-Ups | Pso blog: GetLostGuttate

My first guttate psoriasis flare-up occurred towards the end of 2021, shortly after a particularly sore throat- possibly the most painful one I’ve ever had. I was put on a year-long waiting list for light therapy. In the meantime, I ditched steroid creams in favour of natural skincare, reset my diet and took supplements. By the time I got to the top of the waiting list (which actually took 18 months) I had completely cleared my skin. Around 6 months later- at the beginning of 2024- I came down with another sore throat (along with flu-like symptoms) after which my psoriasis returned. So yes, it appears that I have recurrent guttate psoriasis, too- seemingly caused by throat infections. My light therapy 'slot' was kept open for me, and I start it on Friday. After that's cleared my skin, I've decided that I'm getting a tonsillectomy to hopefully remove the trigger. All the best!
